This is like the “Conway” notation join-lace (L0) operator applied to a square tiling. A model can be made with Antiprism ( http://www.antiprism.com ) using the command
unitile2d -w 6 | wythoff -c L0 | antiview
However, the base L0 tiling pattern needs to be tweaked to create a model with the geometry of the posted model
unitile2d -w 6 | wythoff -p [V,1.414EF]1F,1e1_0e,1_0E | antiview
